Payment/Health Insurance Policy
All payment is due at the time of service. Methods of payment accepted are cash, checks and credit cards (including FSA debit cards). Dr. Gwen does not participate in any managed care plans. All clients will be provided with information needed to file for out of network benefits.

Cancellation
Please notify the office of any cancellations 24 hours in advance of the scheduled appointment.
